Hospice Support

If the resident’s condition warrants the additional services of Hospice Care, additional resources and staff will not increase the monthly rate. Some Hospice residents have remained at Avonlea for years, thanks in part to dedicated support staff who continue to provide services for quality of life.

When Mom or Dad live at home, but the family or caregivers take a vacation or need a rest, our fully-furnished respite “hotel” rooms provide all the services to your loved one as the regular residents.

With your the doctor’s diagnosis of Alzheimer's/Dementia, your loved one will be individually  assessed by our Resident Director of Nursing, and all medications and care plans will be continued by our staff for a minimum of two night stay...up to one month. Your resident’s doctor will remain your point-of-contact, and all meals, ADLs, and individual attention will assure that your loved one feels quite at home with us.

Depression Care

Adult Day Care -- FIRST TO THE SAUK VALLEY !

Adult Day Care – hourly rates with all the benefits of full-time residents. Experience our “DAY OUT Program” for yourself with just a 24-hour notice, once approved.

Fill out the Application, submit provided physician-signed paperwork, and schedule your TOUR. Visit your first time with your loved one.

Your loved ones will be treated as one of the family, staying 2 hours to 8 hours, and can participate in activities and meals, and will be assigned their own room and bathroom while they are with us.

Three reasons we provide quality Day Care:

  • To give the Caregiver a break. We know how hard it is for family to be on-the-job 24/7, and we can provide a safe and engaging environment while you enjoy your “DAY OUT”.
  • To allow the Family to see how their loved one will adjust away from home. See how the home-setting of AVONLEA Cottage makes the difference.
  • We Love Those Living with Alzheimer’s / Dementia, and will do what we can to help them find comfort and hope.

Our RESPITE STAY – Two over-nights up to 30 days. Private room and bathroom, and all meals and activities included.

Grant money available for Day Care and Respite Stays through NIAAA of up to $1000 per qualifying family per year. Contact your local County on Aging for an Application.
